Crockett Memorial Fund Requirements
Qualifications
To be eligible for financial aid from PIN’s Crockett Memorial Fund you must:
Qualify as very low-income or extremely low-income (view income levels here)
Be a resident of San Mateo County or Santa Clara County
Be the legal owner of the animal for whom you are seeking financial assistance
Not have applied for financial assistance from PIN in the last year (based on previous approval date)
Required Documentation
Proof of income (most recent tax return or social security income statement)
Proof of residency (valid driver’s license, lease/rental agreement, recent utility bill, etc.)
Valid identification (driver’s license, state-issued identification, or passport)
Application Procedure
Gather the required documentation to the best of your ability. If you have not been seen by a veterinarian yet, that’s okay!
